Student Parent Registration

Please note there are a couple of steps that you need to complete in order to fully register for the College Bound program. Steps 1 and 2 noted below are required to be completed by all registrants. Step 1 registers you for the program. Step 2 contains three waivers that you must print, complete, and submit by mail by May 31. Step 3 must be completed by anyone who requires medication while attending the College Bound program. If you do not require any medication while in attendance, you do not need to complete Step 3 below. You should complete all three steps (or two, if no medications needed) prior to May 31. • Step 1: Registration
  
  
  • In this section you will be registering for the program. Registration may take 10-15 minutes to complete if you have all of the information at hand. In this section we will be gathering personal and medical information, high school contact information, and college plans. We strongly encourage you to print the PDF of the registration form, complete it, and then use it as a guide as you enter your registration on-line. Please note that once you log into the system you must complete the registration process. You cannot save and return to the system later to update your information. The entire registration process must be completed at one time. Please remember to click on the SUBMIT button at the end of the form so that your registration is submitted. If you do not click on SUBMIT, your registration form will not be received by Virginia Tech.
    
    When you set up the account and register on-line this must be done in the student's name.

• Step 2: Waivers
  There are three waivers that you must complete and return by May 31, 2008. Please click on each of the links below, print the forms, sign, and return to the address indicated.

• Step 3: Medications Form
  
  
  • In this section we will be gathering in depth information regarding medications that the Student may need while they are attending this program. Anyone requiring medication while attending this event must complete this form. If you will not require medication while attending this event you do not need to complete this form. Please fill out this form no later than May 31, 2008. This form will be printed and made available for you on-site to read over and sign.
