The National Health Outreach Conference planning committee with financial support from our sponsor will provide registration, travel, and lodging expenses for five applicants representing successful local health outreach programs to attend and present at the 2016 National Health Outreach Conference, April 6-8, 2016 at Hotel Roanoke, Roanoke, VA. Conference Purpose:

The purpose of the 2016 National Health Outreach Conference is to build a collaborative approach with organizations and agencies to achieve a healthier and safer America by promoting health and wellness, advancing health equity and reducing health disparities. The focus is on the local community environment working toward system change to address health disparities through partnerships at the national, state, and local level. This purpose will be achieved by providing attendees opportunities to explore:

  1. Successful practices that lead to healthy and safe community environments
  2. Applications that help people access clinical and community preventive services
  3. Models for empowerment in health practices for individuals and families
  4. Best practices that lead to addressing health disparities and health literacy in communities and families

Criteria for Selection

  1. Selected participant must represent a local community-based project and program that has enhanced a health or health care initiative that improves access to and quality of care in the community, or broadened the reach of an established program through collaboration with other organizations/agencies achieving a systems approach.
  2. Selected participant must represent a local community-based project and program that reflects the focus of the purpose of the 2016 National Health Conference as identified above.
  3. Participant’ organization does not have the resources to support conference attendance. Travel arrangements and payment will be made by Virginia Tech. Incidental expenses will be reimbursed according to Virginia Tech guidelines.
